Chronic Conditions Management – Change Activities for Your General Practice

Starting 1 July 2025, significant changes to Chronic Conditions Management (CCM) MBS items are set to take effect, marking the first major update in 20 years. To help practices transition smoothly, the Central and Eastern Sydney PHN (CESPHN) is launching a CCM activation series. This series encourages small, manageable changes and aims to strengthen patient connections through MyMedicare.

Activity Highlights:

  1. Patient Engagement: Engage patients returning for Chronic Disease Management Plans and team care arrangements with MyMedicare.
  2. Team Awareness: Raise awareness among practice teams about MyMedicare and the new CCM items.
  3. Practice Preparation: Discuss changes with your team, update workflows, and register for MyMedicare if not already done.

Additional Steps:

  • Prepare practice teams to discuss MyMedicare with patients.
  • Utilise MyMedicare brochures, registration forms, and posters.
  • Update practice websites and social media with MyMedicare information.
